Appointments, referrals and what to expect in our office.

Appointments and referrals

Call our office at 713-960-9700 to make an appointment. If a referral is required by your insurance, it must be provided prior to making an appointment.

Paperwork

Save time by downloading our registration forms and filling them out, as far as you can, at home. Bring them with you to your appointment. If you need to fill out the forms at our office, let us know. Do not come early to fill out paperwork.

In the office

  • Please no food or drink in the office.
  • Children must be supervised by parents or guardians at all times.
  • If you are sick or have been exposed to someone with flu, Covid or another virus, please wear a mask to your appointment.
Having stroke symptoms? Sudden face drooping, arm weakness, or slurred speech is an emergency. Call 911 right away. Do not call the office or wait for an appointment.
