The packet has 14 pages: demographic information, records release authorization, privacy notice receipt, and your patient history. You can type into it on a computer, or print it and fill it out by hand.
Fill out the forms as far as you can at home and bring them with you to your appointment. Call 713-960-9700 if you have any difficulty downloading and printing the packet, and we will arrange for you to fill it out at the office.
Please don’t arrive early to do paperworkIf you need to fill out the forms at our office, let us know ahead of time.
